Abstract
결론
양전자 방출 단층촬영 (Positron emission tomography, PET)은 증례보고 및 문헌 고찰내
용에 따라 구강 악안면 영역의 종양에 있어서 조기 진단 및 경부임파절 전이, 재발 및 잔여
암종의 조기 진단, 방사선 치료나 항암화학요법 치료 후 치료의 경과 및 종양의 예후 판정
등에 유용한 진단방법으로 고려되고 있다. 그러나, PET의 올바른 사용을 위해서는 명확한
검사 목적 및 장기적 계획을 가지고 연속적인 PET검사를 시행하는 것이 중요하리라 사료
된다. 이는 전술한 내용과 같이 동일인에선 연속적인 PET 촬영시 재발이나 치료의 반응과
같은, 종양의 변화 여부를 보다 빠르고 정확하게 진단할 수 있기 때문이다. 또한, 상호 기록
법(co-registration technique)을 이용해 정확한 해부학적 위치를 잡아 보다 정확한 조직검사
등을 통해 초기 암종의 완전절제와 더불어 각각의 PET 기기와 영상 채득 기준에 따른 일
정한 진단 범주를 설정하는 것이 중요하리라 사료된다. 이에 저자 등은 구강 악안면 영역의
종양에 있어서 PET 사용례를 증례보고를 통해 발표함과 더불어 문헌 고찰 등을 통해 얻은
약소한 지견을 보고하는 바이다.

Positron emission tomography(PET) is a diagnostic method that creates high
resolution, 3-Dimentional tomographic images of the distribution of positron emitting
radionuclides in human body. Computed tomography(CT) and magnetic resonance
imaging(MRI) are routinely used to delineate the presence and extent of primary and
metastatic oral and maxillofacial malignancy. Because the CT and MRI criteria for
malignancy depend upon morphology, these imaging techniques have limitations in their
ability to identify malignant tissue when fascial planes are distorted or destroyed. PET
has the advantage of imaging metabolic changes that appear to be linked to malignancy.
Recently, PET has shown promise in the early detection of primary and metastatic oral
and maxillofacial canter and has been used to evaluate the tumor response to
chemotherapy and radiotherapy. We report cases of diagnosis of oral and maxillofacial
cancers and review the literatures to determine whether PET is effective in identifying
oral and maxillofacial tumors.
